The report should include the following information:

• Name of the business
• Type of occupancy classification
• Date of inspection
• Name of business owner/occupant
• Name of person accompanying the inspector during inspection
• Code and edition as a reference
• List of violations and their locations (Code section should be referenced)
• Specific recommendations for correction each violation
• Date of follow-up inspection
